Dear valued customer:
The Capacitance Manometer (CM) product line and associated pressure sensing element assembly (sensor, a CM component) require an export license for export or re-export to certain countries and/or end users as indicated in the US Export Administration Regulations (EAR Part 738 Supplement 1).
A complete listing of countries requiring an export license can be obtained at www.bis.doc.gov.
If Brooks Instrument will be the Exporter of Record for this shipment, we will require the following information in order to submit an application for the export license. These requirements and attachments are only intended to be used with the submittal of purchase orders to Brooks and are not intended to constitute legal advice or to be used as a substitute for specific legal advice from a licensed attorney. You should not act (or refrain from acting) based upon information in this site with obtaining professional advice regarding your particular facts and circumstances.
A. Completion of the Non-Nuclear end user statement on the letterhead of the ultimate consignee as follows
1a. We the Consignee <Full Name and Address of ultimate consignee / End User>
1b. Have ordered from (the Exporter) <Full Name and Address of intermediate consignee / purchaser from Brooks Instrument>
2. Quantity of each model or component ordered
3. Signed by: Printed Name & Title of Signature Authority & Signature of Signature Authority
B. Completion of the Order / Quote screening form. Please provide concise / specific description of the application and how the item will be used.
C. Single submission purchase order OR in the case of an end user expecting to purchase CM on an ongoing basis, Brooks can apply for a blanket license valid for 12-18 months. In this case, please provide the forecasted quantity, product description and unit price
Export licenses take 4-6 weeks for approval from submittal date of complete information
